Registration Policies

The registration form accompanied by a non-refundable fee will reserve a space for your child. Open registration begins each year in March. We do not accept request for specific teachers.

Prior to attending school, the State requires each child to have a physical exam and up-to-date immunizations.

The following forms must be completed by the first day of school:

• Health history (parent’s report)
• Physical and Immunization forms
• Identification and Emergency information
• School contract signed by parent
• Medical consent form
• Parents rights form
• Earthquake Kit
• Change of clothes in Ziploc bag
• Personal Rights form
